Introduced for the 2004 model year, the first-generation Titan was Nissan's bold entry into the American full-size truck market. Built in Mississippi, every Titan came standard with a potent 5.6L Endurance V8 engine, initially producing 305 horsepower, and a 5-speed automatic transmission. Available in King Cab and Crew Cab configurations, it offered innovative features for its time, including the Utili-track bed channel system, a factory spray-in bedliner, and a lockable bedside storage compartment.

- Check the rear axle for leaking differential seals, a common failure. Look for signs of gear oil slung onto the inner side of the rear wheels.
- Listen for a ticking noise from the engine bay, especially on a cold start. The factory exhaust manifolds are known to crack, resulting in a costly repair.
- Inspect the frame and bed supports for significant rust, particularly if the truck has lived in a northern climate.
- Early models (pre-2008) can suffer from a failed radiator cooler that mixes coolant and transmission fluid. Check the transmission fluid for a milky, pink appearance.
